To have the best chance to avoid the glitch you should do all of the missions in your own game as host and turn them all in yourself.
Regarding the glitched achievement, " Moon Mission Meister," it seems this achievement may not unlock if you complete some of the side missions while playing co-op. Killing the Opha Superior is one of the final mission requirements, so you should come across it and kill it, but if you have somehow missed it, DO NOT enter the Vault without first having found and killed it to be sure you haven't missed it. Regarding the potentially missable achievement " Vault Hunter Superior," it should not be missable but some people are reporting that they somehow managed to finish the final mission without ever encountering it. For " Guardian Guardian" you will need to make a choice during a side mission to get the achievement.
